Low sperm count, poor motility, or abnormal morphology of sperm can make it very difficult to conceive using your own sperm sample during treatment. In some cases, even though there are sperm cells in the ejaculate, there might be no live sperm cells or it might be that the existing sperm cells are all abnormal with no fertilization potential. Sperm donation is usually the last resort once the other options have been exhausted. Before proceeding with a sperm donation treatment, it will be important to explore these options and see if they can offer you a solution.

The first step of assessment would be to have a semen analysis done by Kruger method so that the sperm count, concentration, motility and morphology can be checked. Depending on the results of your semen analysis, the sperm sample will be categorized with respect to its count, motility and morphology. In cases where the semen analysis comes back with “azoospermia” as the sperm classification, it will be important to further investigate whether you suffer from Obstructive or Non-obstructive azoospermia. Depending on your diagnosis, surgical sperm retrieval / extraction methods can be an option. In cases of obstructive azoospermia, it is possible that there is sperm production in the testes, but the obstruction in the passage ways prevents sperm cells from reaching the ejaculate. If this is the case, then surgical sperm retrieval can possibly allow us to extract sperm cells via alternative methods and use the extracted sperm cells during an IVF cycle. In most obstructive azoospermia cases, we can successfully extract and isolate live sperm cells.

In non-obstructive azoospermia, there is no sperm production. This can be due to a pathology, a congenital defect or a genetic problem. Regardless of the problem, germ cells fail to differentiate into mature sperm cells and the male patient cannot take advantage of surgical sperm extraction methods since there are no sperm cells inside epididymis or other parts of the testicles. In such cases, sperm donation is usually the only option.

When other options do not produce desired results and a viable sperm sample cannot be found to fertilize the eggs, then sperm donation becomes the only option. Some patients may prefer to directly opt for sperm donation rather than going through the other procedures as using donor sperm will still give the highest chance of success possible.

Sperm Donation is a procedure by which the egg of a female is fertilized, using artificial insemination techniques or IVF, with sperm from a healthy male that has been donated and kept frozen in a sperm bank. The resulting embryo may then develop into a fetus inside the uterus. This way, a couple gets a chance to conceive a child who has genetic traits of one of the parents and the mother can experience pregnancy.

IVF treatment using sperm donation us very much like our IVF/ICSI treatment program. All the steps involved are similar apart from the fertilization stage where instead of the male partner’s sperm sample, sperm from a donor is used.
